Scope:
The Social Worker LBSW will conduct a psychosocial assessment to help identify barriers (social, behavioral, physical, financial) that prevent the patient from being compliant with their established plan of care and will explore alternatives and supplemental payor sources. The Social Worker will develop a plan to address identified barriers and collaborate with other health care professionals and community resource agencies to provide appropriate solutions and resources including assistance with Patient Assistance Programs.

Education:
  • Bachelor’s Degree in Social Work from an accredited program of study. Master’s Degree preferred.

Licensure/Certification Requirements:
  • LBSW (Licensed Baccalaureate Social Worker) required. LMSW (Licensed Master Social Worker) or / LCSW (Licensed Clinical Social Worker) preferred.

Experience:
  • 3 -5 years in a healthcare setting, pharmacy setting or social services agency.  Experience with Pharmacy and Patient Assistance Programs preferred.

Duties and Responsibilities:
  1. The Social Worker will practice within the scope of his/her licensure.
  2. Perform psychosocial assessment to identify social, physical, mental, emotional, spiritual, and supportive needs of the patient.
  3. Develop and implement psychosocial plan of care to provide appropriate resources to address needs.
  4. Identify non-clinical barriers to compliance with established plan of care and develops appropriate solutions utilizing available resources (i.e. transportation problems, language barriers, knowledge deficit and others).
  5. Collaborate with other professionals to evaluate patient’s medical or physical condition and to assess client needs.
  6. Use effective listening techniques to identify where the member is in their stages of behavior change and responds appropriately.
  7. Develop and maintain working relationships with community agencies, providers, and other healthcare workers.
  8. Assist patients with locating various resources within their communities and with Patient Assistance Programs.
  9. Assist patients with developing self-management skills.
  10. Assist patients with coordinating care across the continuum of services.
  11. Encourage the patient to follow the plan of care.
  12. Provides emotional support to patients and families.
  13. Participates as a member of the multi-disciplinary team.
  14. Continuously gather, update and review information to include (but not limited to) collecting psychosocial assessment information.
  15. Maintain documentation in the computer system.
  16. Use good organizational skills to manage time and resources efficiently.
  17. Use effective writing style to organize information and thoughts and present them clearly and concisely in documentation and correspondence.
  18. Take actions upon any awareness of non-medical issues which involve the participant’s safety or welfare and attempt to direct the participant or family to appropriate providers or community resources, or to personally notify appropriate authorities.
  19. Adhere to all company policies as stated in the employee handbook.
  20. Always maintain a professional attitude and approach using tact, courtesy, self-control, patience, loyalty, and discretion to work harmoniously with others.
  21. Maintain the ability to adapt to new situations and changing work responsibilities.
  22. Participate in the Quality Management Program by adhering to all company policies and procedures and identifying opportunities for improvement to ensure quality services are rendered to clients and customers.
The salary range is $60,000 - $65,000 annually. 

Work Environment / Physical Demands:  This position is in a typical office / home office environment which requires prolonged sitting in front of a computer.  Requires hand-eye coordination and manual dexterity sufficient to operate standard office equipment including operation of standard computer and phone equipment.
